Your next chapter deserves continued support.

A transition out of residential treatment should not mean the end of behavioral-health support.

    Understanding your care

    Start with what’s
    happening now.

Lumina Behavioral Health can help eligible clients continue care after completing residential treatment at Lumina at the Manor.

When appropriate, residential and outpatient teams can work together before discharge to develop a more practical transition plan. The goal is to help clients move between appropriate levels of care while maintaining support for mental health, recovery, trauma, daily functioning, and long-term wellness.

    Talk through your options

A practical plan for returning home

    • Telehealth counseling and in-person outpatient appointments
    • Mental-health therapy
    • Trauma and EMDR treatment when appropriate
    • Substance-use counseling
    • Relapse-prevention planning and recovery check-ins
    • Family education
    • Coordination with local providers
    • Support during the transition home
    • Adjustments to the continuing-care plan

    Our approach

Recovery does not occur in isolation

Depending on the individual treatment plan, outpatient and continuing-care services may include family education and coordination in addition to individual counseling, recovery planning, relapse prevention, and coordination with other healthcare professionals.

    Your next step

Coordination before and after a transition

For individuals transitioning from Lumina at the Manor, the residential and outpatient teams can work together to help create a practical plan for returning to everyday life. Professionals can contact our team to discuss clinical needs, available services, care coordination, and potential next steps.

Telehealth availability depends on clinical appropriateness, client location, provider licensure, and applicable regulations.

    Let’s understand what comes next.

You don’t need a diagnosis, a referral plan, or all the right words before reaching out. Start with what is happening today.
