    Clinical needs.
    Connected next steps.

Healthcare professionals often need more than a list of services. They need to understand whether a provider may be appropriate for an individual’s needs, what levels of care are available, how treatment can continue after a transition, and who to contact when coordination is necessary.

Depending on clinical needs and the individual treatment plan, services may include the options listed here. Appointments may be provided in person, through telehealth, or through a combination of both when clinically appropriate.

    Transitions in care

    Continuing care after residential treatment

A transition out of residential treatment should not mean the end of behavioral-health support. Lumina Behavioral Health can help eligible clients continue care after completing residential treatment at Lumina at the Manor.

When appropriate, residential and outpatient teams can work together before discharge to develop a more practical transition plan.

    Care coordination

    Support across the bigger picture

Behavioral-health needs frequently intersect with other medical, psychological, recovery, and social needs. Lumina’s outpatient services may include coordination with other healthcare professionals as part of an individualized treatment plan.

The goal is to help clients move between appropriate levels of care while maintaining support for mental health, recovery, trauma, daily functioning, and long-term wellness.
